Both fasting and carb-restriction appear to operate along similar physiological pathways. Both lower carbs. Both increase fat-adaptation. Both have the potential to get you into ketosis. Both lower insulin and blood sugar.
But is one better than the other? Are there certain scenarios in which an intermittent fasting protocol works better than a low-carb diet, and vice versa?
Let’s find out if the distinction matters.
And what scenarios are most impacted by any difference.
As humans, our most important bodily endowment isn’t our claws, sharp teeth, powerful haunches, iron grips, prehensile tails, venomous secretions, or aerosolized musk. It’s the brain. We use it to shape the world around us, to bend physical reality to our will, to manipulate matter and create powerful technological terrors. These days, the human brain is more important than ever. If you want to enjoy life, pursue and succeed at your passions, to conquer your little corner of reality—you need a healthy brain. Brain health is key to total health—and quality of life.
By some analysis at least, however, neurogenerative diseases remain on the rise and take an ever more extreme emotional and economic toll. So, how do we keep our brain health intact? While much of it comes down to doing the things that keep your brain healthy and avoiding the things that harm it—exercising instead of sitting on the couch, breathing exclusively fresh air instead of tobacco smoke, sleeping instead of staying up—another big variable is the food we eat.
My staff and I are quite close. Things stay busy these days, so there isn’t a lot of downtime, but I’ve worked with some of these folks for over a decade. We don’t discuss every grisly detail of our lives with each other. But we do share. We care about each other.
So when one of the Worker Bees mentioned he was having some potentially serious medical issues, I asked for details. Turns out he went to his doctor for a hard lump on his throat that was getting progressively bigger. Initial pokes and prods were inconclusive. An MRI led to a biopsy, which led to an email in the middle of the afternoon with the results and a hell of an opener: “This may be a cancer.” May helped. It wasn’t a sure thing yet.
For today’s edition of Dear Mark, I’m answering three questions from readers. I’ve come down hard against phones in bedrooms in the past. Is there a “good way” to use your phone in the bedroom? I can’t complain about my existence in modern culture. My life is great. I have a loving family. My kids are happy and successful. My wife is a friend and lover and confidante and partner. Business is good and interesting. I care about what I’m doing. Every day is meaningful—and unburdened by concerns around mental well-being. Depression isn’t an issue for me.
But it’s not the case for everyone. The numbers don’t lie. Depression rates are climbing. Antidepressants are among the most common drug prescriptions, even among children. And because it can be embarrassing to admit you’re depressed—like there’s “something wrong” with you if you say as much—many people with depression never seek help, so the real numbers could be even higher. Depression isn’t new of course. The ancients knew it as “melancholia,” or possession by malevolent spirits. But all evidence suggests that depression is more prevalent than ever before.
What’s going on?
With 6.1 million children in this country bearing a past or present ADHD diagnosis, it’s little wonder folks had a lot to say in the initial post I did on the subject a few months back. For the most part, people were pumped to discover new potential therapies for themselves or loved ones, or at the very least to find validation in their own hunt for side effect free ADHD treatment. Others questioned the validity of certain alternative approaches, and still more posed questions about other treatments they’d heard about or were interested in.

Over the course of a day’s hike or in a sudden wonderstruck moment, many of us have felt the edges of our selves dissolve into the wild that surrounds us. We become unconsciously “of” our environments. Shedding the insular, constraining cages of our everyday hyperrationality—the mental chatter, the rigid expectations, and inevitable tension and failures that accompany them—identities and desires evaporate into the senses. For a time, we become raw awareness. The heightening of the senses alone can feel like a kind of animalistic thrill.
I haven’t talked much about stress this month, and I don’t want to give it short shrift. Yes, there’s a lot to take apart with food and exercise, both of which can feel more “actionable” at times. But stress can be a major roadblock to success. How we deal with emotional and physical stress will invariably impact our health, well-being and performance. Until we dial it in, we’ll compromise the results of all our other Primal efforts.
I’ve said in the past that stress has been one of the hardest aspects I’ve struggled with—and continue to now and then. Living Primally means I’m running on full rather than empty to be sure, but that doesn’t mean the rest of the world always conforms to logic or sanity, let alone my preferences. And emotional tension aside, I like to push myself periodically in the gym or on an outdoor adventure, which means I’m dealing with physical stressors, too.
Here’s one thing I’ve done for twenty years to counter both emotional and physical stress.
A few months back, I put together an article on alternative therapies for depression. Many readers showed a lot of interest in some of the emerging non-drug treatments in that field, and, more importantly, many began to relay their own stories about how they overcame or successfully managed their depression through various strategies. That’s what I love the most about this community—sharing experience and expertise in the interest of broadening available solutions beyond what conventional thought tells us.
I’ve received frequent requests over the years to do the same for the convoluted world of ADD and ADHD treatment. There’s a lot to this picture and (if there’s interest) probably fodder for a follow-up post. For today I’ll delve into some of the lifestyle strategies and alternative therapies that offer the most promise.
