I use Food journal for the Paleo Diet - PaleoTrack to keep track of carbs, calories and etc. If you can't find an actual book, how about starting your own journal and putting in foods as you find their values so that you can then keep your daily record?
As to tracking, write everything in daily, and I do mean everything. It's amazing how much a handful of nuts a day can affect your totals over time if you don't record them.
