    Lipid Screen

    All:

    Well, I guess I'm the newest member of the "You need to go on statins" club. My doc gave me the results of my recent lipid screen:

    TC = 187
    HDL = 46
    LDL = 130 (107 by the Iranian calc)
    Trigly = 57

    He made me laugh when he mentioned I might want to start taking Lipitor...

    For an interesting datapoint - when I was diagnosed on 9/15/11 with Type 2 Diabetes (now in remission) my triglycerides were 1100!

    Now, he also did the NMR test - this much I know:

    Particle Conc = 1746
    Average Size = 20.8

    He's mailing me the results. Any comments about the NMR data?

    You have a TC of 187 and he wants you on statins?!? Incredible...

    I'm sure the only thing the doc saw was LDL at 130, which can be out of range (max range 100 for some labs, 130 for others).

    Definitely seems ridiculous to pay doctors for these robotic interpretations.

    Thanks for the advice - this is the first time this Doc has let me down. He was more than helpful when he took me off the Januvia/metformin/water pill. He knows he's going to get a lecture when I see him again. He's waiting to be educated on the NMR LDL test...

    Ok, here's the updated version of my lipid screen - my doc is nice enough to have the NMR lipid profile done:

    Total Cholesterol 187
    LDL-C 130
    HDL-C 46
    Triglycerides 57
    LDL Particle Number 1746
    Small LDL-P 834
    LDL Part. Size 20.8
    HDL-P 30.5
    Large HDL-P 4.3
    Large VLDL-P <0.7 (not measurable)
    LP-IR Score 30 (insulin sensitive)

    Note that my blood glucose was 500+ and my A1c was 11 on 9/15/11. Tryglycerides were 1100.
    Started 9/15/11 at 323 pounds
    2/16/13 at 241 pounds
    Goal is 223 pounds or less

    Cholesterol Ratios are what really matter.

    According to the ratio calculations, here's how yours read:

    Total/HDL: 187/46 or 4.065 (ideal = 5 or below)

    Trig/HDL: 57/46 or 1.24 (ideal = 2 or below)

    LDL/HDL: 130/46 or 2.826 (ideal = 4.3 or below)

    Point being, you are on the right track and statins are whack.

    One of the kind of "lost" parts of Taubes work "Good Calories, Bad Calories" was the study on the relationship between TC and likelihood of a CHD related death. Basically between around 180 and 240 there is NO correlation between TC and the # of CHD related deaths. For MALES, from 180 TC as the TC does down, the risk of CHD goes UP and from 240 TC as the TC goes up the risk of CHD goes UP.

    For females, especially post menopausal, the risk of CHD actually goes DOWN as TC goes up from 240.

    The studies cited are pretty old and really only focus on TC, which even most CW know that there's more to the picture than that. But, Point is that (at least from a TC standpoint) at 180 the biggest risk *might* be if you TC drops a little an gets into the below 180 range. However, I think that risk is mitigated a lot by the solid HDL number.

    If a doc's talking statins at 187, he's obviously a "cook book" type doc and you need to look for something better. He probably quotes BMI numbers like gospel as well.
