    Primal Fuel


    My doctor told my father to take statins just wondering what your opinion is on his cholesterol profile.


    Total cholesterol: 358

    Triglycerides: 206

    LDL: 272

    HDL: 86


    Total/HDL: 4.16

    Trig/HDL: 2.39

    LDL/HDL: 3.16


    It seems to me his ratios are fine but something else is causing a rise in the total cholesterol. Any idea what this might be or if he should be worried about atherosclerosis?

    Best thing for him to do is lower those triglycerides - and that means cutting his carbs down to the bone. The HDL is great, and he should have his LDL pattern checked to see if it's A or B. If it's A, he's fine. That means taking a VAP test.

    With those trigs, the LDL is definitely way higher than it should be and almost certainly mismeasured. Under the Iranian formula, it's only 160.

    Regardless, the ratios are good either way:
    With Calc LDL value:
    Total/HDL = 4.1 where 4.4 is ideal for women and 5.0 is ideal for men
    Trig/HDL = 1.0 where 2.0 is ideal and lower is better
    LDL/HDL = 2.9 where 4.3 is ideal and lower is better

    With Iranian LDL value:
    Total/HDL = 4.1 where 4.4 is ideal for women and 5.0 is ideal for men
    Trig/HDL = 1.0 where 2.0 is ideal and lower is better
    LDL/HDL = 2.5 where 4.3 is ideal and lower is better

    I'd say those are pretty good numbers!

    The doctor should be smacked. I assume that LDL was calculated. Have the doctor do a VAP test - is the male in question on the PB or a similar LC diet? Here's the difference between a calculated-by-Friedewald LDL and a calculated-by-Iranian LDL:

    Friedewald: 144
    Iranian: 127

    The Iranian formula is more accurate than the Friedewald one. Additionally, direct measurement of LDL particle size would be instructive. That said, here's the ratios (which are easy, folks - just divide the first number by the second number):

    Total/HDL: 226/66 = 3.42 where 5 or less is optimal for a man
    Trigs/HDL = 71/66 = 1.08 where 2 or less is optimal
    LDL/HDL = 144/66 = 2.18 where 4.3 or less is optimal (or 1.90 with the Iranian equation)

    Trigs/HDL indicates low heart disease risk. What is his doctor smoking? Tell the doctor ABSOLUTELY NOT to statins (and demand that he get up to speed on the current research about cholesterol, especially fat intake and HDL, ratios, and LDL particle sizes).
