I think it's always a good idea to eat real food (no toxic grains, no vegetable oils, NO SUGAR) and avoid the processed food that most young people seem to depend on. As for whether to follow a paleo/primal diet hardcore, I think that depends on someone's goals... is it to solve health problems? lose weight? cross a fitness plateau? just overall health? and does the kid/teen really want to do it, or are you a parent trying to force them to?
I wish I had started the PB as a teen. My health would only have benefited from it.

Why would it be that a standard diet of grains and processed food and sugar that we all know is bad for us, would be appropriate for children? Why would it be that the food that helps adults recover from obesity and diabetes and promotes general health and wellbeing would be harmful to children?
You lousy kids! Get off my savannah!
In utero.
My friend has a Primal 2-year-old. Never been sick, ever.
Same for Sarah at everydaypaleo.com. Her littlest one is probably about 4 now. Never been sick.
If you have other kids already, then you can appreciate how remarkable that is.
