    Stale means not explosive, weights don't bounce slightly at the end of a rep, and reps are slow and grinding. Feels like failure is coming. It's something you get a feel for after a long time of programmed lifting.

    Very rarely. Generally when I'm squatting over 400 or deading around 500. Or when I'm doing an intentional overload week, or I feel that lower back fatigue is a sole limiting factor in a lift that doesn't work it specifically(like overhead press[what belts were invented for]).

    If I use a belt, I note it in my notes.

    I keep expecting to hit a wall, but it just doesn't happen. It's like I'm doing the same workouts over and over(as far as exertion) but the weight keep going up. Muscle mass has gone up noticeably, while leaning out- keeping weight about steady.
