I may have a touch of carb flu, but I can't figure out what Mark wants me to do for the self assessment! Am I supposed to do the essential exercise for each type (regular push up, regular pull up, etc) and use that number to determine where I start? If so, why does Ken Krog do dive bombers to determine his over head press number?

go ahead and try the regular exercises if you can handle them. if you find that you can't do dive bombers or pull ups or something, just try the same exercise on level 3, and keep going until you find a level that is just beyond comfortable for you. don't be surprised if you're all over the place in what you can do...you might find yourself at level one for the overhead press, level five for squats, and somewhere in between for the others.
