  • 2 months Primal and just got labwork from physical



    ..and my cholesterol is very high! First time in my life.

    I've NEVER had problems in this area, I am not overweight, it does not run in the family...

    I can only attribute it to this 'diet'. Yes, I am failry strict, but eating more meat that I used to.

    think I am going back to just fish, and retain some pricipals, but I see that this much meat/bacon/saturated fat really can't be that good for you.

    I have no other idea why this could be?

      Ok-I have the labwork at home...and I will post tomorrow/tonight.

          High cholesterol is not bad. Depends on the numbers and where each one is. Unfortunately, most physicals don't do the NMR profile which gives you the important information. Hopefully when you post the numbers they're included but I doubt they will be. Usually it has to be specifically requested, and usually when it is requested it is met with a blank stare.


          I know Rickard's site (free the animal) has done extensive posting on this topic, and Dr. Davis (heartscanblog) has as well.

              Ok here are the numbers


              CHOL fasting 286/mg/dl


              HDL fasting 134


              triglcerides 41


              Direct LDL 129


              Chol HDL 2.1


              what is going on here? 286 seems very high? and it was normal/low before 'primal'

                Wow, your HDL is really high and your Trigs really low, exactly how it should be. LDL is usually calculated and not measured. One is the Friedwald formula, which I believe is generally used by labs, and the other is the Iranian Formula which gives a better estimate for people on low carb diets. When I plugged in your values, your LDL value using the latter formula turned out to be 102. I'm sure Griff can explain the ratios better.

                  Your HDL being that high is *wonderful.* Don't worry about the total number - with an HDL like that, you're in great shape. Here's something most doctors won't tell you: the ratios are more important than the actual numbers.


                  As maba said, if your LDL was calculated, not measured, the calculation (known as the Friedewald equation) doesn't work when trigs are under 100, as yours are. It overestimates LDL. Since your list says "Direct LDL" I am going to assume that your LDL was directly measured and not calculated, but just for reference, here's what would happen if it was calculated:


                  The formula used to calculate total cholesterol is: LDL + HDL + [Trigs/5] = total. But what they usually do is calculate your LDL. They measure your HDL, your Trigs and your Total - so the equation becomes: Total - (HDL + [Trigs/5]) = LDL.


                  This calculation works fine for trigs over 100. But when your trigs drop below 100, it becomes misleading and usually calculates your LDL far higher than it should be. Always, always demand a VAP test, which is a direct measurement of the LDL. According to Dr. Mary Vernon, "These labs in which the LDL is calculated are not accurate if your triglycerides are below 100... The equation used to calculate these numbers makes assumptions which are not accurate when triglycerides are low." (from http://www.livinlavidalowcarb.blogsp...t-results.html).


                  Now, about those ratios.


                  The ideal ratio of Total/HDL is 4.4 for women and 5 for men. Also, according to http://wwww.yourmedicaldetective.com/public/532.cfm and several other sites, the ratio of your trigs to your HDL will indicate whether your LDL is small and dense (bad) or large and fluffy (neutral). A larger number indicates smaller LDL particles and a smaller number indicates larger LDL particles. It's an inverse relationship.


                  Your Total/HDL ratio is 286/134, or 2.1. That's fantastic.


                  The ideal ratio of Trig/HDL is 2 or below. 4 is high. 6 is "danger!!" Yours is 41/134, or 0.31 - that's not just fantastic, that's incredible. It's one of the best I've ever seen.


                  Additionally, a low ratio of Trig/HDL is great because it's a semi-reliable indicator of lower free insulin levels. Lower free insulin is good. However, this doesn't appear to work for those of African descent, so take that with a grain of salt.

                  Primal eating in a nutshell: If you are hungry, eat Primal food until you are satisfied (not stuffed). Then stop. Wait until you're hungry again. Repeat.

                    WOW thank you SO much for these detailed and informative replies...I'll be sure to check out those links also.

                    I was SO discouraged when I saw the numbers and the so called 'normal' range, and the Drs recommendation to eat less fat/saturated fat. Previously I had not been eating hardly any meat at all (I mean, like 3x/year at most!) and was afraid this shift caused the numbers...so glad to hear that this is all good!

                    as I was shocked by this. THANKS!

                      Also - just did more research on this - the Iranian equation mentioned in the comments on Dr. Mike's blog (last link in my previous reply) says that your LDL was overcalculated. This equation does a better job of estimating LDL through calculation even when trigs are below 100.


                      The Iranian equation is: (Total/1.19) + (Trig/1.9) - (HDL/1.1) = LDL


                      According to the Friedewald equation, your LDL is 144. According to the Iranian equation, it's 102 - a 42-point difference.


                      One other thing I read is that the large, fluffy, innocuous LDL (pattern "a" LDL) is heavier than the small, dense LDL (pattern "b" LDL), and when they measure cholesterol, they measure by particle weight in the sample. So even though you might have more cholesterol by weight left over when the HDL and trigs are separated out, the particle size assumption may not be correct. Remember that as trigs go down, fluffiness of LDL particles goes up - which means they weigh more, and if they weigh more, the measurement is going to be inaccurate.

                      Primal eating in a nutshell: If you are hungry, eat Primal food until you are satisfied (not stuffed). Then stop. Wait until you're hungry again. Repeat.

                          yes thanks again!

                          I was quick to panic--saw those numbers compared to the 'recommended' along with the Drs recommendations...and of course jumped to conclusion that what I am eating is bad as its the only thing that changed.


                          I'm a bit miffed that even some simple googling I did supports all the info here...and frustrated the Dr would just say-cut saturated fat.

                            I agree with what has already been said, high hdl and low triglycerides are as good as it gets!!


                            It is a shame to think that there are doctors out there who don't know this.
