Why isn't lactose included? How does it compare to glucose and fructose?

Not included because if you're strictly Primal you eat no dairy or low dairy. Butter and raw full-fat cheese are preferred choices and both are very low in lactose.

    Lactose is 50:50 glucose:galactose.

Galactose produces a bigger insulin spike than glucose. Hence lactose is best avoided for anyone who is insulin resistant.

For those who are insulin sensitive, milk and yoghurt are excellent post-workout foods, as they promote muscle growth and repair.
